North Kirklees Clinical Commissioning Group – updates from Winter 2015 Newsletter

North Kirklees CCG Winter 2015 newsletter is out.  Some of the key points highlighted are:

Care closer to home Earlier this year they awarded a contract for a range of community-based services to Locala Community Partnerships. Over the past few months, Locala has been working with their staff, patients, GPs and others to implement their new model of care across Kirklees.

‘My health tools’  is an interactive website designed for people living with long-term conditions in Kirklees. Developed by Kirklees Council, NHS Greater Huddersfield CCG and NHS North Kirklees CCG, the site is designed to help people manage long-term conditions and improve their health and wellbeing. Users are taken on a guided journey to identify their personal health needs and provided with support to help them manage and monitor their condition. Initially focusing on pain, breathing problems and anxiety, the site will develop over time to include a range of topics and health conditions. My Health Tools is easy to use, open to anyone who feels they would benefit and can be found here www.myhealthtools.uk

Local services – your views count – over 400 responses were received to a recent survey about local GP services. The feedback you provided is now being used to inform the development of our new strategy for GP services, which will be published in the New Year.

Christmas Pharmacy opening times – can be found here.
